A – Exchange Your Existing
OnSite
for a Reprogrammed
OnSite |

OnSite until the new reprogrammed AED arrives
- You are never without an AED

To exchange your
OnSite for a reprogrammed AED, please read the following: |
- If you purchased your
OnSite on or after November 28, 2005, there is no charge for this service.
- If you purchased your
OnSite before November 28, 2005, the cost is $280 for each AED, however you will receive a free
2-year extended warranty, a
$395 value!

Upon receipt of your order form, Philips will
ship you a reprogrammed OnSite in approximately 5 to 10 business days. Then, you will return your
OnSite in the same box.
IMPORTANT:
DO NOT RETURN YOUR BATTERY, PADS AND CASE.
Please note: Philips can only
accept your OnSite for exchange if it is undamaged. This
ensures that all reprogrammed AEDs going back out to
customers are in like new condition. If the OnSite you
return is damaged or shows signs of wear, the charge for
the exchange will be $590.

B – Return Your
OnSite
to Philips for Reprogramming
|
|
Use
this option if keeping the same AED you purchased is
important (i.e. markings, stickers, or asset management
and tracking).
Please
note:
With this option, you'll be without your OnSite for
approximately 3-4 weeks.

- Philips will return the
same serial number for asset tracking
|
|
To return
your OnSite for reprogramming, complete this
order
form. Upon receipt of your AED, Philips will
reprogram and ship the same OnSite in approximately three weeks.
Note: You will be
without your Onsite for approximately three weeks.
